Ok, many comments here but so far I haven't got much further in my understanding of the problem.
From what I see there are two notable differences between your setup and the configurations used by many here:
  • The power supply.
  • The case and cooler.
I think people suggest the official power supply not because that's the only thing that works, but to establish a working baseline from which you can identify what was wrong with your original setup.

From what you've reported, it seems the PMIC is burned out and that's why the I2C failure.

As mentioned earlier, the active cooler

directly makes contact with the PMIC to keep it cool. My impression is this active cooler is more popular than the official case with fan, especially with people running 100 percent workloads.

In addition to overheating, a PMIC related failure could be related to the quality of 5V power.

Thus, swapping out the power supply and using the better cooling solution seems like a first step for tracing your problem. If after making these two changes the Pi 5 continue to fail, that's interesting. If they become reliable that's also interesting.

Edit: One more thought following the thread

The PMIC could be overheating with fan off in the official case when the Pi is shutdown. To fix this one might set

POWER_OFF_ON_HALT=1

in the firmware when using the official case.

The active cooler may radiate enough heat open air to avoid PMIC damage in power on halt when the fan is not spinning. The official case less so.
